At lot has been said and done about the LFI process. We often work in close partnership with clients to improve their Incident Management approach. Next to training, coaching and supporting where needed, we also help them to take the final step. In our view even a very high quality incident investigation is pretty useless if the results and insights are not followed up properly. A lot of value is lost here!

Therefore we support clients in learning from incident investigations by: 

• Conducting meta/ trend analyses over multiple investigations

• Follow-up on recommendations

• Assess barrier strength in underlying Risk Assessments

• Making use of IncidentXP insights in BowTieXP
