Tripod Beta is a ‘reactive’ method for incident analysis leading to proactive process control measures. It does not just focus on the immediate, human, causes of the incident, but identifies the structural flaws that allowed the incident in question to occur. The Investigation framework clarifies unknowns and uncertainties and thus leads to insights. Underlying causes are identified and lead to well considered conclusions and management actions, strengthening the organisation’s sustainability as well as business and safety performance.

Human Error Theory 

Human error means that something has been done that was "not intended by the actor; not desired by a set of rules or an external observer; or that led the task or system outside its acceptable limits". In short, it is a deviation from intention, expectation or desirability. Logically, human actions can fail to achieve their goal in two different ways: the actions can go as planned, but the plan can be inadequate (leading to mistakes); or, the plan can be satisfactory, but the performance can be deficient (leading to slips and lapses).However, a mere failure is not an error if there had been no plan to accomplish something in particular.

Tripod Beta Practitioner Training

Our Tripod Beta  Practitioner Training teaches participants the Tripod theory and approach, and how to investigate and analyse operational disturbances. In addition, the supporting software facilitates the analysis of, and reporting on operational disturbances by the investigation team. It helps to explicitly identify underlying causes and take corrective action, improving process and safety performance. We organize both PUBLIC and IN-COMPANY training courses.

Tripod Beta Accreditation

Upon completion of the Tripod Beta Practitioner Training, attendees can opt to go through a formal Tripod Accreditation process as facilitated through the ‘Stichting Tripod Foundation’ (www.tripodfoundation.com), which is linked to the Energy Institute.

Tripod Beta Software

Participants will be provided with a trial license for the IncidentXP software, which will remain valid for 30 days after activation. This software is the leading platform for incident investigation & analysis, combining over 5 different investigation methodologies in addition to Tripod Beta.
